Birth Registration/Corrections/Changes

SUMMIT COUNTY PUBLIC HEALTH OFFERS OHIO BIRTH, DEATH CERTIFICATES

Summit County Public Health now offers certified birth and death certificates copies for all Ohio counties for any person who was born or died in Ohio.  This covers births from Dec. 20, 1908, to the present and deaths from January 1, 1971, to the present. The cost is $22 per copy.

Summit County Public Health is located at 1867 West Market Street. They can be reached at 330-812-3845.

Prior to December 20, 1908, all live births were registered in the Probate Court in the county where the birth occurred. For births before December 20, 1908, contact that Probate Court to get a copy of the registration.

Requirements to Correct, Change or Record a Birth Certificate:

Ohio law provides that a person who claims to have been born in Ohio, and whose registration of birth was not recorded, has been lost or destroyed, has not been accurately recorded, or requires a change to gender marker, may register, correct or change their birth record by following the required steps listed here.

These records will be maintained confidentially and not visible to the public. Depending upon the application and the Affidavit evidence, the Court may require a hearing.

Procedure for Correcting Birth Records:
** To be used if there is a clerical error in the birth record, for example a name is misspelled.

Procedure for Changing a Gender Marker on Birth Records:
** To be used to change the gender assigned on the birth certificate.

Procedure for Delayed Registrations of Birth:
** To be used if the birth was never registered with Vital Statistics.
