SUMMIT COUNTY PUBLIC HEALTH OFFERS OHIO BIRTH, DEATH CERTIFICATES
Summit County Public Health now offers certified birth and death certificates copies for all Ohio counties for any person who was born or died in Ohio. This covers births from Dec. 20, 1908, to the present and deaths from January 1, 1971, to the present. The cost is $22 per copy.
Summit County Public Health is located at 1867 West Market Street. They can be reached at 330-812-3845.
Prior to December 20, 1908, all live births were registered in the Probate Court in the county where the birth occurred. For births before December 20, 1908, contact that Probate Court to get a copy of the registration.
CAREFULLY READ THE FOLLOWING INSTRUCTIONS TO SAVE YOU TIME
Requirements to Correct, Change or Record a Birth Certificate:
Ohio law provides that a person who claims to have been born in Ohio, and whose registration of birth was not recorded, has been lost or destroyed, has not been accurately recorded, or requires a change to gender marker, may register, correct or change their birth record by following the required steps listed here.
These records will be maintained confidentially and not visible to the public. Depending upon the application and the Affidavit evidence, the Court may require a hearing.
Where to File:
- If born in the State of Ohio, file in the Probate Court of the county of birth, the county of residence or the county in which the mother resided at the time of birth.
YOU MUST DO EACH OF THESE THINGS FOR ANY TYPE OF APPLICATION:
- You must file the appropriate application as designated below and attach a certified copy of the birth certificate to be changed unless the birth was not registered.
- You must file two Affidavits which meet the requirements for the type of order you are seeking. The Affidavits must be completed by individuals who are not the applicant and who have personal knowledge of the applicant’s requested correction, change or birth in Ohio. All signatures must be notarized.
- The applicant must come to the Records room of Probate Court (no appointment necessary) and pay court costs at the time of filing.
- If the person for whom the correction, change or registration is a minor, only a parent or legal guardian may apply; applicant’s signature must be notarized.
Procedure for Correcting Birth Records:
** To be used if there is a clerical error in the birth record, for example a name is misspelled.
- In Probate Court file an application [1] for a Correction of Birth Record. Attach to the application a certified copy of the birth certificate to be corrected.
- At least two (2) Affidavits must be completed by individuals who are not the applicant and who have personal knowledge of the birth. (All signatures must be notarized).
- Evidence must be presented that verify the correct facts of the birth. At least two (2) pieces of acceptable evidence are required. (Please see list of acceptable evidence [2]).
Procedure for Changing a Gender Marker on Birth Records:
** To be used to change the gender assigned on the birth certificate.
- In Probate Court file an application [1] for a Change of Birth Record. Attach to the application a certified copy of the birth certificate to be corrected.
- A Licensed Professional Statement [3] and one (1) Affidavit completed by an individual who is not the applicant and who has personal knowledge of gender status. (Signature must be notarized).
- OR – two (2) Affidavits. They must be completed by individuals who are not the applicant and who have personal knowledge of gender status. (All signatures must be notarized).
- Evidence must be presented that verify the correct facts of gender status. At least two (2) pieces of acceptable evidence are required. (Please see list of acceptable evidence [2]).
Procedure for Delayed Registrations of Birth:
** To be used if the birth was never registered with Vital Statistics.
- In Probate Court file an application [4] for a Delayed Registration of Birth.
- Attach to the application a letter of search stating that no birth record can be found. If the birth was prior to December 20, 1908, the letter must be from the Probate Court of the county where the birth occurred. If the birth was after December 20, 1908, the letter must be from The Ohio Department of Health, Office of Vital Statistics in Columbus, Ohio.
- Evidence must be presented that verify the facts of the birth. At least three (3) pieces of acceptable evidence are required. (Please see list of acceptable evidence [2]).
