To Adopt a Person 18 years of age or older
Because of Ohio Law and the complexity of the process it is best to consult an attorney. It is important to note that the Court cannot give any legal advice so an attorney will be the best guidance for you and your family’s particular situation.
When filing for an Adult Adoption:
- All forms must be typed.
- Use full birth-names and no initials.
- If there is no middle name or initial, use (NMI) in the spaces provided.
- Make sure all signatures are included on all forms.
- All documents, forms and fees must be submitted at the time of filing.
- You will be notified by mail of the final hearing date after all of the information is reviewed and approved.
Information needed when filing an Adult Adoption:
- Bring to Court a certified copy of the adult adoptee’s long form birth certificate. When the adoption is final, the original birth certificate will be sealed. The adoptee may decide to change their name at the time of filing. If so, there is no additional charge for the name change.
- If the adult adoptee was in the permanent custody of an agency when they turned 18, the adoptee must provide the Court with proof of their independence.
At the time of filing, the Adopting Parents must provide:
- Birth Records – A certified copy of birth certificates for both the petitioner and the spouse. These documents will be verified and returned after finalization.
- Marriage Records – Certified copy of the Marriage License. This document will be verified and returned after finalization.
- Divorce Records (if applicable) – A Certified copy of the divorce decree for all divorces for both parents. These documents will be verified and returned after finalization.
- Four letters of reference – from non-family members with original signatures. These letters should show support for the parent-child relationship that has been established.
Fill out and file these forms:

- Form 18.3 – Consent to Adoption (signed by adult adoptee and notarized)
- Form 19.0 – Petition for Adoption of Adult
- Form 19.7 – Supplemental Adoption Information
- Form 19.8 – Certification of Adoption Filing
- Form HEA 2757 – Certificate of Adoption
- Form Misc 02 – Self Representation Acknowledgement, if applicable
Filing Fee:
- The filing fee is $159.00.
- If you use a credit card, a 3% charge will be added.
Make checks made payable to the “Summit County Probate Court.”
If the family has a current foster/adoption license, please contact the Adoption Clerk at the Court. Additional paperwork may be required at the time of filing.
