Unless otherwise provided for by Order of the Court, an applicant for release of assets pursuant to Section 2111.05 of the Ohio Revised Code shall:

1. File an application for release of assets without appointment of a guardian. Upon approval of the application, the Order of Release shall state the institution acting as depository, shall state that the funds shall not be released until the ward reaches the age of eighteen (18), or the ward's death, or until further Order of the Court;
   
   
2. Deposit funds in a bank or savings institution; and
   
   
3. File a report within fourteen (14) days after deposit that the deposit was made with the same restrictions on withdrawal as cited in the Order of Release.
